What Is a Community Health Needs Assessment?
A Community Health Needs Assessment (CHNA) is a tool used by public health agencies and hospitals to understand the biggest health concerns in a community. The goal? To promote health equity, reduce barriers, and improve overall well-being—with community voices leading the way.
The Visioning Session: Where It All Began
On March 12th, 2025, the Dent County Health Center (DCHC) and Salem Memorial District Hospital (SMDH) launched the 2025 CHNA with a Community Health Visioning Session. This event brought together residents, health professionals, and community organizations to set a shared direction for the future of health in Dent County.
