In order to provide our services, we may need to collect your personal information, including, but not limited to, name, date of birth, ethnicity, race, gender, address, phone number, social security number, insurance information, and email address.
What will we do with it?
We use this information to keep health records on you. Certain information is required to bill your insurance, such as your SSN. We will also use your cell phone and email address to send you electronic appointment confirmations and reminders. An email address is required in order to request appointments online.
